You'll need to provide:

• Your name and date of birth,
• an email address,
• a contact number and address,
• evidence of identity:
  • copies of TWO forms of ID (one must be photographic), AND
  • an identity referee's written statement OR a clear photograph of you holding your photographic ID. (See 'evidence of identity' for more information).

Note:

• This form is for people who wish to query a formal warning that they were issued by police. These exclude written traffic warnings, verbal warnings and harassment notices.
• Your query form may be used as a legal document, so it’s important to be as truthful, and as accurate as possible.
• The query form cannot be saved to complete later.
• The query form will timeout after one hour of inactivity.
• The information you provide about yourself or someone else can help us. It may also be used for other permitted purposes.
